Please post anime recommendations here, using the format I provided. Also try to refrain from spoiling series, when giving your opinion. I have yet to watch a series worthy of a perfect 5.
Title- Madoka Magica
Description- After experiencing a bizarre dream, Madoka Kaname, a kind 14-year-old girl, encounters a magical creature named Kyube. Madoka and her friend Sayaka Miki are offered the opportunity of gaining magical powers if they agree to make a contract with the strange little being. He will also grant them one wish, but in exchange they shall risk their lives by accepting the responsibility of fighting witches. Invisible to human eyes, witches are catalysts of despair in the areas they inhabit. An ally of Kyube, a magical girl named Mami Tomoe, befriends and encourages the two girls to accept the contract. For an unknown reason, another magical girl named Homura Akemi is determined to prevent Madoka from accepting the deal.

Opinion- This series twists the concept of Magical Girl, and breathes new life into it. It's possibly the best psychological thriller, I ever seen. Ending could've been better though.

Description- In a world rife with deadly creatures called "youma", a young silver eyed woman, Clare, works on behalf of an organisation that trains female youma halfbreeds into warriors with the ability to destroy these creatures. Considered a rogue for picking up a stray child & almost losing herself to her youma side by "Awakening", she is constantly assigned rather dangerous missions...

Opinion- Overall a good series. Only drawback is the ending, that defers from the manga.

Title- Death Note
Description- Yagami Light is an ace student with great prospects, who's bored out of his mind. One day he finds the "Death Note": a notebook from the realm of the Death Gods, with the power to kill people in any way he desires. With the Death Note in hand, Light decides to create his perfect world, without crime or criminals. However, when criminals start dropping dead one by one, the authorites send the legendary detective L to track down the killer, and a battle of wits, deception and logic ensues...

Opinion- The first season was outstanding. Second one not so much. Dull characters came into play, and it seems rushed.

Title- Selector Infected WIXOSS
Description- Hope, desire, greed. Girls with each of these feelings in their hearts are being swallowed into the vortex of a dangerous game...

Opinion- So far, so good. Just wish the card game was a little more interesting. The psychological aspect is off the charts.
Rating- 4.2
Title- Akatsuki no Yona
Description- Yona is the sole princess of the kingdom, living the luxurious and carefree life as a princess should. She has it all: the finest clothes and cosmetics, the most divine sweets, a loving emperor as a father, and the hottest cousin crush, Soo-won, anyone could ever hope to have. Now, if only her bodyguard, Son Hak, wasn't so annoying to her and her hair wasn't so red. But her nearly perfect world quickly shatters as the man she loves, Soo-won, murders her father and the path to his ascension to the throne is assured. Son Hak escapes with Yona, and they lead a life on the run from that point on.

Opinion- Not your run-of-the-mill anime. I can already tell it has a lot of potential.
